Request for Student's or Borrower's Taxpayer Identification Number and Certification (Form W-9S)

Educational institutions and student loan lenders use the W-9S form (Request for Student’s or Borrower’s Taxpayer Identification Number and Certification) to collect a student’s or borrower's Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN).

This information is necessary for reporting tuition payments (Form 1098-T) and student loan interest (Form 1098-E) to the IRS. The University of Minnesota requires students to submit this form to ensure compliance with IRS regulations for education-related tax benefits.
