Permission to Enroll

This form is used for students to request permission to enroll in a UMR course when any of the following permission types are needed:

• Late Add
  • Depending on the time of the request, you may also be required to seek permission from the course instructor, or both the instructor and the Student Scholastic Committee.
• Enroll based on prerequisite course(s) taken at another institution (transfer evaluation)
• Enroll without having taken prerequisite course(s)
• Enroll in a course that requires departmental or instructor approval
• Enroll as a non-Major in a BSHS-restricted course
  • BSN, Twin Cities, or other Multi-I students who wish to enroll in Rochester courses should select this option. 
• Enroll in a course for a third time
  • University policy currently allows students to repeat a course only one time (excluding course withdrawals).  Students who enroll and complete a UMR course or an equivalent course from another UofM campus two times cannot enroll in the same course or its equivalent across the system for a third time.
  • These requests are rarely approved. You should review the guidelines to determine if this petition is appropriate to submit. 
  • This petition will require an explanation that needs to provide the following:
    • Reasons why you were unsuccessful the first two attempts. 
    • Reasons why you need to enroll in the course for the third time.
    • Reasons why you cannot enroll in an equivalent course outside of the University.
  • Petitions for courses that do not meet major requirements will not be considered.
