This form must be completed if you are a borrower under the Nurse Faculty Loan Program in order to claim entitlement to loan cancellation.
Note: Section 4 must be completed by an authorized official from the student’s place of employment.
Use this form if...
- You are a borrower under the Nurse Faculty Loan Program and want to cancel the loan.
Don't use this form if...
- You do not qualify for NFLP cancellation or you do not have a NFLP loan.
