Academic Policy Petition

This form is used to obtain an exception or other accommodation to college or University academic policies for all Undergraduate programs. Your request will be reviewed by the Student Scholastic Standing Committee and other University offices, as necessary.

Use this form if...

  • You are an active undergraduate student who wishes to obtain an exception to the University academic policies.

Don't use this form if...

  • You are not an undergraduate student.
  • You can successfully complete the requested action through MyU.

Next steps

If you are petitioning for medical reasons, have your medical provider or the Disability Resource Center complete a Medical Supplement to document these circumstances. You will be prompted to upload support documentation on this form.
